Information we request may include your name, address/phone number/e-mail, age, cause of kidney problem, years of education, and your attitudes or opinions about RMS Lifeline. Required information is indicated by an asterisk*. Provision of all other information is optional. We may also collect information about you and your use of our site through the use of Cookies. "Cookies" are small computer files that we transfer to your computer's hard drive that allow us to know how often someone visits our site and the activities they conduct while on our site. This information helps us to monitor how many people are using our site and for what purpose, so we can assess the impact of the RMS Lifeline program, and adjust our activities to better suit the needs of patients and professionals.

How we use your information
When you contact RMS Lifeline, we enter your name and identifying information into our private database to allow us to track requests and trends. RMS Lifeline does not release information gathered on the RMS Lifeline website to any outside parties. Your name and information will never be sold; we will contact you only if you indicate a willingness to talk to us. If you make a comment, it will not be connected with your full name or identifying information.

Survey and evaluation information is put together with the responses from hundreds of other people and used for research purposes, to help us improve our efforts.
